By RSVP’ing to attend the 2022 USOPP Partner Summit, for good and valuable consideration, the receipt of which is hereby acknowledged, I hereby agree as follows: I agree to be filmed, videotaped and photographed, and to have my name, image, picture, likeness, voice and biographical information (collectively “Appearance”) otherwise recorded, in any media, by the Los Angeles Organizing Committee for the Olympic and Paralympic Games 2028 d/b/a LA28 (“LA28”) and its official photographers, film crews and video crews, and by any other entity authorized by LA28 (“Footage”), in connection with the 2022 USOPP Partner Summit and any other related events occurring on October 25 and 26, 2022 (including but not limited to any media availability, venue tour and/or youth sports site visit). I also hereby acknowledge that LA28 owns all copyrights and other proprietary rights in and to the Footage, and I hereby grant LA28 and the United States Olympic and Paralympic Properties, LLC (collectively with LA28, the “LA28 Entities”) and their assigns the irrevocable, perpetual, worldwide right and license to use, and to authorize third parties to use, in all media now known or hereafter devised, my Appearance in the Footage for any legitimate purpose, without the need to seek any further consent from me. I further agree that the LA28 Entities and their assignees and licensees shall have all right, title and interest in any and all results and proceeds from usage of the Footage.

In order to help prevent the spread of COVID-19, anyone who is feeling unwell, exhibiting COVID-19 symptoms and/or was recently exposed to someone with COVID-19 in the days leading up to the 2022 USOPP Partner Summit, should not attend. All attendees should understand that COVID-19 is extremely contagious and that there is an inherent risk of exposure to COVID-19 in any place where people are or have been present, and that no precautions will entirely eliminate the risk of exposure to, or infection from, COVID-19. By RSVP’ing to attend the 2022 USOPP Partner Summit, I hereby agree to comply with all health and safety protocols established by the event organizers and all applicable laws or regulations.
